Summary: Under the supervision of an RN performs basic patient care activities in patients' homes.
Job
Responsibilities:
Assists with procedures ordered by physician and supervised by RN or LPN
Tending to patient's personal care, activities of daily living and transfers/transport
Reports and documents abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff
Assists with keeping the patient's environment clean and orderly
Other duties as assigned.
Education: High school diploma or GED.
Required
Experience: One year clinical experience as an aide in an acute, rehab or skilled nursing setting. Home care experience preferred.
Licensure/Certification/Registration:
Certification and Registered with the State of Ohio Nurse's Aide Registry
Current Health Care Provider CPR Certification from American Heart Association
Valid Ohio driver license
Proof of automobile insurance with $100,000/$300,000 coverage
Physical Requirements:
Must be ambulatory and able to drive to patients' homes
Able to climb stairs, pull, push, and facilitate movement of the patient
Medium Work
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects
Physical Demand requirements are in excess of those for Light Work
